HERE’S HOW TO GET STARTED

Provide the following:

¨  Proof of Age

Acceptable documents: birth certificate, baptismal certificate, copy of record of baptism, notarized or certified and showing DOB, notarized statement from parent, passport, prior school records showing DOB

¨  Proof of Residency (for applying parent/guardian)

Must provide 2 forms of proof of residency

Acceptable documents: a deed, lease, current utility bill (within 30 days), current credit card bill, property tax bill, vehicle registration, driver’s license, DOT identification card

¨  Student Physical and Immunization Records

Acceptable documents: Child’s immunization record, written statement from the former school district or a medical office that required immunizations have been administered, or that a required series is in progress. Overview of child’s last well visit.

¨  Student Enrollment and Registration Application

Make sure to provide a valid email address and phone number.

¨  Income Verification

Acceptable documents: 4 paystubs, W2, tax returns, child support documents, etc. (for all household members)

¨  ELN Data Fields Form

Provide the following if applicable:

¨  Zero Income Letter

¨  Student Disciplinary Record

¨  IEP (Individual Education Plan)
